Clinical Nurse Coach (Bilingual)

Novus Health is seeking an experienced and compassionate clinical nurse who is interested in joining a dynamic company that provides and develops health navigation solutions, as well as wellness and health concierge products. We are seeking customer-service oriented professionals who want to grow with us in an evolving industry and who have a global vision of health care. As a clinical nurse coach, you will perform assessments of members’ mental and physical condition and provide continued assistance and guidance on various health issues. Ideal candidates will be nurses who demonstrate a strong patient and family-centered approach that aims to provide the patient’s motivation towards positive behavior change.

Main Responsibilities:

  • Build on motivational interviewing techniques aimed at helping members develop self-care plans.
  • Work as health coach and nurse case manager for various programs within the organization (Coaching and MSO programs).
  • Develop and maintain coaching plans related to members’ needs including, but not limited to, the following: Health Risk Assessment (HRA), lifestyle management, and self- management of chronic diseases.
  • Manage Medical Second Opinion (MSO) cases from start to finish with the support from Novus Health’s Medical Director.
  • Performing a comprehensive clinical assessment of needs and providing the appropriate responses, information, and resources.
  • Maintain records in various health navigation programs.
  • Build commitment among members through open and respectful communication in order to help members understand the Canadian health care system as well as the medical and social aspects of various health conditions.

Other Responsibilities:

  • Use research skills to ensure our health communications and information are accurate, up to date and effective.
  • Work closely with the team of Health Information Specialists (HIS) to ensure the services and material delivered to customers is pertinent and reliable.
  • Contribute to a dynamic work environment by helping optimize processes and customer service.
  • Assist the team in the development and review of health-related content.

Required Skills and Experience:

  • Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses.
  • At least 5 years of clinical experience in various settings, including mental health support solutions.
  • Strong written and verbal communication skills in French and English.
  • Fosters teamwork.
  • Comfortable working in an environment with changing variable work pace.
  • Demonstrates a high level of initiative and independence in self-directed work.
  • Feel comfortable setting priorities.
  • Demonstrates strong critical thinking abilities and solid clinical judgement.
  • Empathic, patient, and attentive.
  • Proven administrative abilities with strong MS Office skills.

Compensation for this role is between $65,000-$70,000 per annum.
